Florida Comfort Choice offers long term care and coordination of services that can help individuals stay at home while improving their quality of life.
The Florida Comfort Choice Long-term Care Community Diversion Program is sponsored by the Florida Department of Elder Affairs (DOEA). Its goal is to keep you out of a nursing home -- and in your community -- as long as it’s reasonably safe. If you can no longer receive care at home, and your doctor feels that a nursing home is a better choice, you may still be covered with Florida Comfort Choice, depending on Medicaid requirements.
Florida Comfort Choice professional care managers and a network of providers are ready to help meet your needs. A care manager works closely with your primary care physician (PCP) to coordinate your care, arrange for social services, member education, and any other needs.
